Not all COVID testing is the same — which test should you trust?

There are two different types of tests: a diagnostic test which will show if you have an active coronavirus infection, and an antibody test which detects antibodies the immune system makes to fight the virus. Of the different diagnostic and antibody tests available, the Centers for Disease Control & Prevention (CDC) and Federal Drug Administration (FDA) consider the PCR/DNA test the most accurate and the gold standard for COVID-19 testing.

PCR/DNA TESTING Highly Recommended ★

The PCR/DNA test is a diagnostic, molecular test that detects the virus’s genetic material or DNA. The specimen is collected via saliva or a nasal swab. Of all available tests, the PCR/DNA test can confirm with the highest accuracy if there is an active coronavirus present. It cannot determine if there has been a coronavirus infection in the past. ANTIBODY TESTING Highly Recommended ★

This diagnostic, serological blood test detects antibodies the immune system makes to fight the virus. Its results are available within 15 minutes. However, antibodies can take days or weeks to develop and may stay in blood weeks after recovery, so this test should not be used to diagnose an active coronavirus infection. It is not known if the presence of antibodies means the patient is immune to coronavirus in the future.

ANTIGEN RAPID TESTING Not Recommended

The antigen rapid test is a diagnostic test that detects specific proteins on the virus’s surface. A sample is taken with a nasal or throat swab and results are available in an hour or less. While its positive results are highly accurate, this test has a 50% miss rate for showing false negatives and cannot be used to rule out an active coronavirus infection.
